Safety Concern/Issue

Please use this form to report problems observed on the campus of Carleton University only.

This form provides a means for informing the Department of University Safety of chronic community problems or conditions that are not of an emergency nature. This initiative is designed to be an effective, proactive response to community concerns.

For instance, a member of the community might use this form to report person(s) roller-blading/ skateboarding in the tunnel systems or inside buildings on-campus. Basically, any non-emergency problem or condition that the Department of University Safety should be made aware of can be reported using this form.

Once the Department of University Safety receives a report, the information is forwarded to the attention of officers that will be patrolling that specific area or building. The information reported in the complaint is recorded by the individual officers in their patrol notebooks and are responded to by patrol officers between calls for service.

Following the initial response, the complainant may be contacted and a follow-up review initiated. If, following this review, the complaint has been definitively resolved, the case is considered closed. If, however, the problem or condition still requires further attention, the complaint will be re-instituted and may be the subject of additional, specifically tailored problem-solving measures aimed at satisfying the need for final resolution of the problem.

Incomplete or inaccurate information will delay processing of this report and may compromise our ability to quickly and effectively respond to this complaint. We may contact you for further information that might aid us in attaining a long-term solution to this problem. Please be aware that the Department of University Safety may share information relating to the problem with other University departments when the potential solution involves intervention by other service providers (i.e.: Building Authorities, Maintenance Services, etc.)
