Are you a new employee?Visit Human Resources to fill out the necessary forms. You are eligible to receive payment once this information has been submitted. |
What to Bring
- A blank cheque or a Direct Deposit Authorization Form (available from your bank)
- Your Social Insurance Number (SIN) card
- Proof of date of birth – bring government-issued photo ID (examples: Driver’s License, Passport)
Full-Time/Continuing Staff
- You will complete all relevant forms in your orientation session
Casual Staff and Graduate Students
You must complete the following forms:
- Personal Information Form
- Personal Tax Credit Return Form – Federal ( TD1) - Use the TD1 – Worksheet for help
- Personal Tax Credit Return Form – Provincial (TD1ON) – Use the TD1ON – Worksheet for help
- Confidentiality Agreement
- Mandatory Training Acknowledgement
- Health and Safety Employee Orientation Manual (last page)
Employees hired from international locations
You must also provide:
- Work or study permit / VISA
- Passport
- Proof of University Health Insurance
