Benefit Changes
If you would like to change your benefit information, please fill out the applicable form(s) and submit to the Benefits section of Human Resources.
To see your current benefit plan details, go to the Employee Services section on Carleton Central and click on Benefits and Deductions.

| Beneficiary Nomination | Change your beneficiaries for Group Life Insurance (Sunlife). |
| Evidence of Insurability – Coverage Detail | This form is used in addition to the Group Coverage Change form when you are a late applicant for health, dental or optional life insurance, or when you want to increase your optional life insurance. |
| Group Coverage Change | Enrol in or change benefits information – includes health & dental, reinstatement or refusal of benefits, optional life insurance, beneficiary changes and name changes. |
| Optional Life Insurance – Trustee Appointment | Assign a trustee for optional life insurance when your beneficiary is a minor or lacks legal capacity. |
| Pension Beneficiary Change | Change your Pension Beneficiaries. Contact the Pension Officer if you require assistance. |
