If a Work-related Injury or Illness Occurs
- Provide and record first aid given
- Fill in the Supervisor’s Incident Investigation Report Form. This must be submitted to Environmental Health and Safety within 2 days of the incident.
When Further Medical Attention is Needed
- Arrange for medical treatment of employee(s) and provide the necessary transportation
- Complete the Treatment Memorandum Form and WSIB Functional Abilities Form and give them to the employee
- Complete the appropriate sections of the WSIB Form 7 and return it to the Benefits Coordinator in Human Resources within 24 hours of notification of an accident or illness
- In the event of a critical injury or exposure to infectious disease, additional steps are required. Refer to the Environmental Health and Safety’s Injury Reporting and Investigation webpage for more information
Reporting Obligations
- The injury must be reported to the Workplace Safety & Insurance Board (WSIB) within 3 days if the worker obtains medical treatment and/or misses time from work
- The WSIB will levy a penalty of $250 for a late submission of a Form 7
- This penalty will be charged back to your department
- Please advise your employees of the importance of reporting workplace injuries / illness immediately to you following an accident or the onset of an illness
