Workplace Injury or Illness



If a Work-related Injury or Illness Occurs

  1. Provide and record first aid given
  2. Fill in the Supervisor’s Incident Investigation Report Form.   This must be submitted to Environmental Health and Safety within 2 days of the incident.

When Further Medical Attention is Needed

  1. Arrange for medical treatment of employee(s) and provide the necessary transportation
  2. Complete the Treatment Memorandum Form and WSIB Functional Abilities Form and give them to the employee
  3. Complete the appropriate sections of the WSIB Form 7 and return it to the Benefits Coordinator in Human Resources within 24 hours of notification of an accident or illness
  4. In the event of a critical injury or exposure to infectious disease, additional steps are required.  Refer to the Environmental Health and Safety’s  Injury Reporting and Investigation webpage for more information

Reporting Obligations

  • The injury must be reported to the Workplace Safety & Insurance Board (WSIB) within 3 days if the worker obtains medical treatment and/or misses time from work
  • The WSIB will levy a penalty of $250 for a late submission of a Form 7
  • This penalty will be charged back to your department
  • Please advise your employees of the importance of reporting workplace injuries / illness immediately to you following an accident or the onset of an illness
