Health and Dental Benefits – Data Review

The Human Resources Department and Great West Life are undertaking a joint process of verifying the data contained in Carleton’s BANNER system against the data within Great West Life’s in house system in terms of coverage under the Extended Health Care plan and the Dental plan.

As part of the process, you will receive a letter at the beginning of February 2013 asking you to confirm your dependent information. Our Group Insurance provider (Great-West Life) requires this information to process claims correctly. It is therefore critical for Great-West Life to have your accurate dependent information on file.

The letter you will receive will list your current dependent information. You will be asked to review the information for accuracy and completeness and indicate any required changes to your dependent information. Once the form is complete, you will be required to return it to Great West Life no later than February 15, 2013. A pre-paid envelope will be provided.
